Nomination Forms

There are many opportunities for women to represent their congregations and conferences within the state and national synodical organizations. Some positions require documents be submitted as part of the selection process.

Voting Member Credential Registration Form for the Fall Gathering (writable PDF). This form must be submitted to the conference registrar no later than October 15.

Triennial Voting Member Nomination Form (writable PDF). Use this form to nominate an eligible woman to attend the Triennial Convention as a voting member. The Triennial Convention will be held on Zoom beginning in 2026.

SWO Board Member/Officer Nomination Form (writable PDF).  Use this form to nominate yourself or an eligible woman to serve on the SWO Board or as an Officer.
